Lessons Learned: Parting of Lifeboat/Tender Fall Wire

Transport Malta experiences of the parting of lifeboat/tender fall wire in place on a passenger ship.

What occurred?

On 29 October 2021, while the passenger was at anchor at Cabo San Lucas, Mexico, lifeboat/tender no. 11 was lowered and launched at sea.

Whereas heaving up the davit’s floating blocks with out the boat, the ahead fall wire broke, inflicting the block to fall into the water. No accidents had been reported.

The protection investigation concluded that the autumn wire sustained tensile overload, resulting in the parting of the wire.

Contemplating the actions taken by the vessel’s managers, the MSIU has issued no suggestions.

Actions taken

Through the security investigation, the Firm took the next security actions:

the interior investigation report was shared with the officers and engineers on board and the teachings discovered fashioned a part of subsequent hand-over notes;
the producer’s pointers on the operation of the lifeboat/tender had been reviewed by the upkeep group;
the launching procedures and steerage posters had been displayed on all lifeboats’ winch touchdown areas;
all upkeep jobs had been required to be communicated upfront and reviewed/authorised by the employees captain and the chief engineer;
the protection officer was required to seek the advice of all departments on the launching, restoration, and upkeep operations.
