Lessons Learned: Equipment Failure and Subsequent Damage to Vessel

Transport Malta stories gear failure and subsequent injury to a rescue boat in Port Everglades.

What occurred

At about 1003 (LT) on 14 April 2021, whereas one of many rescue boats was being hoisted again to its nesting place after a dynamic check on the davit winch brake had been accomplished, the wire snapped. The boat fell into the water from deck no. 5. The vessel was moored alongside at Port Everglades, Florida.

The boat was finally recovered. Not one of the crew members suffered any accidents, however the rescue boat sustained structural damages to its hull.

The MSIU discovered that though the hoisting button on the winch management had been launched, the winch didn’t cease and finally, the metal wire failed.

Microscopic evaluation of the contactor revealed indicators of worn and welded contacts ideas. Thewire rope’s situation didn’t contribute to the accident.

The MSIU has made two suggestions to the Firm addressing lifeboat davits ancillary gear.
